Senior Client Care Manager
Sequoia Community Initiatives, a non-profit organization committed to serving the needs of New York City’s homeless, has an immediate opening for a Senior Client Care Manager. The Senior Client Care Manager will coordinate outreach, intake and client orientation components of the program. Supervise Client Care managers for 24 hour client care coverage.
Duties:
Serve as liaison between clients and clinical staff.
Monitor the implementation of the care plan to ensure that programs are followed
Conduct intake interview, orientation of clients to facility rules and regulations
Coordinate schedules with other professional services and care teams
Assist clients with adjustment to shelter living with periodic follow-up
Motivate the client towards the goal of permanent housing
Resolve client inquires and disputes
Conduct substance abuse groups and workshops for clients
Maintain log book documenting all activities in the shelter on the assigned shift and noting any variance to the normal day to day operation
Maintain head count; monitor the census and placement of clients to assure compliance with DHS rules and regulations
Plan and organize field trips, house meetings, and special activities for residents and staff
Monitor the physical upkeep of the facility
Manage and oversee all aspects of the facility in the absence of the Program Director and/ or Social Services Director |
